What Is a D-SNP Plan?
D-SNP stands for Dual Eligible Special Needs Plan. "Dual eligible" refers to individuals who qualify for both Medicare (the federal health insurance program for people 65+ and certain individuals with disabilities) and Medicaid (the joint federal-state program for people with limited income and resources).
D-SNPs are a specific type of Medicare Advantage plan — which means they replace Original Medicare and deliver all Part A and Part B coverage through a private insurance company. But D-SNPs go further by coordinating Medicare and Medicaid benefits into one plan, reducing complexity and often delivering additional benefits that standard Medicare Advantage plans don't include.
What Benefits Do D-SNP Plans Include?
Benefits vary by plan and location, but many D-SNP plans in 2026 include some or all of the following:

Do You Qualify for a D-SNP?
Basic Eligibility Requirements
- You are enrolled in Medicare Part A and Part B
- You are enrolled in your state's Medicaid program (any level of Medicaid)
- You live in the plan's service area (plans are county-specific)
- You do not have End-Stage Renal Disease (ESRD) — though rules changed in 2021 and ESRD patients may now enroll in some plans
If you receive any form of Medicaid — including full Medicaid, QMB (Qualified Medicare Beneficiary), SLMB (Specified Low-Income Medicare Beneficiary), or other Medicare Savings Programs — you may be eligible. The specific Medicaid category affects which D-SNP plan you can enroll in and what additional benefits you receive.

When Can You Enroll in a D-SNP?
This is one of the most important advantages of D-SNP plans: most allow monthly enrollment. Unlike standard Medicare Advantage plans that typically restrict enrollment to the Annual Enrollment Period (October 15 – December 7) or special enrollment windows, many D-SNPs accept new enrollees throughout the year — one of the key benefits of being a Special Needs Plan.

Why Most People Who Qualify Don't Know About D-SNPs
There are a few reasons:
- Medicare itself doesn't proactively connect the dots. Medicare and Medicaid are administered separately, and neither program automatically enrolls you in a D-SNP even if you qualify for both.
- Many brokers don't specialize in dual-eligible clients. If your broker mostly sells to Medicare-only clients, they may be less familiar with D-SNP availability in your area.
